Indications
Sildenafil is indicated for the treatment of erectile dysfunction in men and pulmonary arterial hypertension in men. In pulmonary arterial hypertension, concomitant therapy with sildenafil is recommended. In patients with cardiovascular risk factors, sildenafil should be used with caution. Use of sildenafil should be part of a comprehensive treatment plan for pulmonary arterial hypertension. If appropriate, sildenafil should be used alongside other pharmacotherapy in patients with erectile dysfunction. If sildenafil is not available, several other pharmacotherapy are recommended. If sildenafil is not in use, sildenafil is used with caution in patients with certain cardiovascular risk factors. In patients with moderate to severe pulmonary arterial hypertension, sildenafil is indicated as first-line therapy. Sildenafil should not be used to treat priapism. Sildenafil is not indicated in men with severe heart failure. Sildenafil should be used in combination with appropriate other pharmacotherapy as treatment of pulmonary arterial hypertension.
Dosage and Administration
Adults and adolescent women: The dosage of sildenafil in the treatment of erectile dysfunction should be decided individually based on cardiovascular risk factors (see section 4.8). Erectile dysfunction in men has been reported to occur in a small percentage of Viagra-naive women (ranging from 10 to 40%). There is also the possibility of an association with pulmonary arterial hypertension (see section 4.4).
Patients with cardiovascular risk factors (e.g. hypertension, heart failure, previous myocardial infarction) and those with severe cardiovascular disease should be offered sildenafil. Sildenafil should be used with caution in patients with certain cardiovascular risk factors (see section 4.5).
Adults
The treatment of erectile dysfunction should be based on cardiovascular risk factors. The usual starting dose of sildenafil in the treatment of erectile dysfunction in men is 25 mg, taken approximately one hour before sexual activity (see Table 1). When sildenafil treatment is started at the lowest dosage level (that is, the lowest dose level that is able to maintain sexual arousal for a minimum of one hour), the effects of sildenafil should be closely examined (see section 4.8). If there is no improvement in sexual desire, it is recommended that the dosage level be increased to 50 mg (see Table 2).
The usual starting dose of sildenafil in the treatment of erectile dysfunction in women is 50 mg, taken approximately one hour before sexual activity (see Table 2). When sildenafil treatment is started at the lowest dose level (that is, the lowest dose level that is able to maintain sexual arousal for a minimum of one hour), the effects of sildenafil should be closely examined (see section 4.8). If there is improvement in sexual desire, it is recommended that the dosage level be increased to 100 mg (see Table 3).

The recommended starting dose in the treatment of erectile dysfunction in women is 100 mg, taken approximately one hour before sexual activity (see Table 4). When sildenafil treatment is started at the lowest dose level (that is, the lowest dose level that is able to maintain sexual arousal for a minimum of one hour), the effects of sildenafil should be carefully examined (see section 4.8). If patients have changes in sexual desire, it is recommended that the dosage level be raised to 100 mg (see Table 5).
